Peripheral Vascular ABI

An Ankle-Brachial Index (ABI) test is a quick, noninvasive procedure used to diagnose peripheral artery disease (PAD) by comparing blood pressure measurements at the ankle and the arm. The ABI ratio helps clinicians identify reduced blood flow in the lower extremities caused by arterial narrowing or blockages. ABI testing is commonly performed in vascular labs, cardiology offices, primary care settings, and wound care clinics.

During an ABI examination, Doppler ultrasound probes and blood pressure cuffs are used to measure systolic pressures at multiple sites. The ankle pressure is divided by the brachial (arm) pressure to calculate the ABI value. A normal ABI falls between 1.0 and 1.4; values below 0.9 indicate arterial disease, while values above 1.4 may suggest calcified, non-compressible arteries. Many modern ABI devices also offer segmental pressure testing, pulse volume recording (PVR), and toe-brachial index (TBI) measurements.

Consider ABI testing for patients with the following conditions: claudication (leg pain with walking), non-healing wounds or ulcers of the lower extremities, diabetes with peripheral neuropathy, chronic kidney disease, hypertension, hyperlipidemia, history of tobacco use, coronary artery disease, stroke or TIA, and age 65 or older with risk factors for PAD.
